Thanks for all the advice on here as I too am looking to book flights for next year to include our yet to arrive grandchild.
I've been really really pleased by the response I got from one airline,unfortunately it was only the one who replied, Monarch. So they'll get our booking. As a tired old cynic I am surprised any of them did but shocked by the fact that it is all good news. Not only did they say,,,"You'll be glad to know that the good old days are not completely gone, as you can still book the infant with the first name as "baby" and the surname as the family name.Once the booking has been confirmed, you can provide the name of the infant to our call centre by e-mail or by phone and we can amend the name of the infant free of charge".
But when I thanked them I also quietly moaned about the 0871 number which is so awkward and expensive to ring from Spain (Mind you Jet2 who are impossible to get a reply from have an 09 number which is a fortune to ring) they said this....
Dear Jamie, Thank you for your reply. The 08719 number is still in play. However, we do have a free phone number from Spain. It is only valid on landline. The number is 800099260.
..well I wasn't expecting that. Even better when I said I would let the people on Babyexpert know this good news I asked if it was possible to have an email contact as that is even more convenient for busy Mom's / parents/ Grandparents and I mentioned that people didn't like speaking to centres around the other side of the world and they replied.....

I hope this helps with anyone wanting to book flights in advance of their newborn arriving not just for Spain but I have used Monarch to other destinations too. Good service and customer relations 1 - poor service and rip offs Nil .

I feel much better reading everyone's response to the above question as I am looking to book a holiday with Thompson ASAP for next summer, I have a 3 year old and I really wanted a free child place for her, to keep cost down!! I know you don't really pay for baby on flights & accommodation. (I think you still have to pay tax on baby's flight I know I did with Ryanair when my daughter was under 2.) but not sure how is works for other airlines.
I think I will give Thomson a call to see if this is still the case, to note baby on booking as "infant" or "baby" & "EDD" as DOB, then obviously call them back to give them the correct details after baby has been born - would be a shame to miss out on the holiday with a free child place!!!
