Do you have PCOS by any chance? That can make OPK’s unreliable.

It's possible that your body gears up for ov with an LH surge but the egg doesn't actually get released. Are you still doing OPKs or did you stop after the + ?

I am thinking the same as Gillsy. Last cycle I had EWCM around the time I "should" have OV'd (i.e. around day 14), then nothing happened. I ended up ov'ing on day 41, which actually if I'd had a normal 28 day cycle would have been the right timings for cycle 2 if that makes any sense (in that if I'd ov'd and had a 28 day cycle I would have ov'd on day 13 the next cycle)? It's almost like my body tried to ov but didn't quite manage it.

As above, and echo the question did you continue to do an OPK?
On my last cycle, I got true positive OPKs for 3 days running (and very close to positive on a 4th day). As you ovulated 12-36 hours after the surge, it's positive you have a long surge so you ovulation could occur 3 days later?

hiya, i do have PCOS
and i have 2 clear blue readers so i carried on doing them and got no smiley after the event. i also do IC and had a strong line but not quite positive, then the next day a faint line again

Keep up with the temping, your body might try and ov again in the next few days.

Keep going with the temping. OPKs can be brilliant but you can also have a surge with no actual ovulation, temps are the only way to be sure you are actually ovulating. I did OPKs for quite awhile and when i then started temping i found the two didn't always match up but i went with the temp chart and conceived both months i did this.
